Types Of Dental Implants

Endosteal Dental Implants

Endosteal (which means within the bone) dental implants are placed into the jawbone and are completely surrounded in bone. Typically made of hardened titanium and shaped like root-form screws, they are the commonly used type of dental implant. Endosteal dental implants are the most successful medical device ever placed in humans.

Dr. Bramanti only uses dental implants that have a proven scientific record of success in humans for over 40 years. Having placed over 16,000 dental implants in his clinical career, Dr. Bramanti brings an unmatched clinical expertise, experience and superb record of dental implant success spanning over 37 years.

Mini Dental Implants (toothpick dental implants)

Mini Dental Implants are narrow toothpick-sized dental implants which are narrower and weaker than most commonly used endosteal dental implants. They are placed through less-invasive techniques and are used primarily to stabilize a lower denture or bridge. Mini Dental Implants have a short lifespan as they tend to fail quickly (within a few months to a few years) due to adverse bite loading and infection. The Mini Dental Implants are just too narrow and weak in structure to support the normal bite forces placed on a denture or bridge.

Although Mini Dental Implants are promoted as a cheaper alternative to endosteal dental implants, your investment in them is short-lived and lost due to the high level of Mini Dental Implant failure, fracture and patient frustration. Dr. Bramanti cautions you not to choose this path for denture or bridge stability. Endosteal dental implants are the proven standard for lifelong success.

Subperiosteal (under the gums)

Subperiosteal dental implants are a metal framework placed under the gum, fitted to the jawbone. This type of framework implant has been used in the past in patients who do not have enough healthy natural jawbone and cannot, or do not want to, undergo a bone augmentation procedure to rebuild it. Subperiosteal dental implants were originally placed because endosteal dental implants were not yet available in this country. Unfortunately, subperiosteal dental implants generally have a short lifespan and frequently fail due to exposure and infection.

Additional Dental Implant Techniques

Depending on the health of your jawbone and your specific needs, Dr. Bramanti may suggest some alternative treatment options in addition to the traditional multi-step dental implant procedure. Options may include:

If your jawbone is not able to support dental implants, several bone augmentation techniques can be used to rebuild bone, restore your natural jawline and provide a sturdy foundation for implant-supported teeth.

Guided Bone Graft Augmentation

This involves restoring or regenerating bone in your jaw when it is lost and insufficient to support dental implantation. Scientific research shows that using bone graft additives and growth factors to fortify the bone, in conjunction with bone graft coverage using a collagen membrane, generally achieves the best predictable results. After a healing period of 6 months, the bone deformity is repaired by the guided bone graft regeneration permitting the subsequent placement of dental implants into the newly created bone.

If your jaw isn’t wide enough to support dental implants, bone graft material can be added to a small or narrow ridge using the guided bone regeneration method described above. Bone graft ridge augmentation develops sufficient width and/or height of your jawbone to support a dental implant. Once the graft is mature (6 months), dental implants can be placed, and teeth restored (see below).

Dr. Bramanti is a world-renown expert in guided bone graft augmentation having played an instrumental role in its development in the early 1990’s. Dr. Bramanti has perfected the technique to such an extent that there are few situations in which he cannot grow bone and reverse a bony deformity or loss of jawbone. If you have suffered jawbone loss or have a bony deficiency, contact Dr. Bramanti to determine how he may best serve you. Dr. Bramanti can offer you unmatched treatment possibilities with his skills, experience, knowledge, and unequaled success to give you the opportunity to regain your bone.

3D Imaging And Treatment Planning

Experienced Board-Certified Dental Implant Expert Specialists, like Dr. Bramanti, do not place dental implants without proper dental imaging. Cone Beam Computerized Tomography (CBCT) imaging accurately identifies and assesses existing bone volume. This state-of-the-art, highly precise 3D digital imaging and implant surgical planning software have made dental implant procedures faster, more precise and highly predictable.

Your Board-Certified Dental Implant Expert can use these planning tools to analyze the anatomy of your jaw and determine the best sites, depth, and angulation for successful dental implants before placement. CBCT-scan imaging allows Dr. Bramanti to precisely plan and virtually simulate the implant placement. This approach to dental implant planning removes guesswork, results in precise dental implant positioning, saves time and money, and shortens recovery time.

This advanced digital approach permits 3D site planning for bioengineering of bone and gum tissues. This includes bone grafting for sinuses (see below) and deficient bony ridges or bony deformities (see below).CBCT planning determines all possibilities for proper implant positioning, precise alignment, and improves the surgical approach from a “mental navigation” to a virtual simulation.

Benefits of CBCT Digital Planning

  • Recognizes and minimizes risks and potential complications in advance.
  • Defines in 3-dimensions the esthetic and functional demands for implants.
  • Identifies and reduces potential surgical risks and complications.
  • Results in accurate predictable results that are stable over time.
  • Final esthetic results are significantly improved providing a natural beauty.

Dr. Bramanti has pioneered CT imaging usage in Fresno in combination with dental implant planning software since 1995. His command of the digital workflow and experience in dental implant planning over the last 28 years, is unlike anything you have ever experienced. You will be able to make a more confident treatment decision regarding your bone graft and/or dental implant needs after Dr. Bramanti reviews with you his digital virtual planning and recommendations for an affordable dental implant solution.

CBCT-Rendered Dental Implant Positioning Guide

Precise dental implant placement is achieved using planning software combining the CBCT data with a digital scan of the proposed position of the final teeth determined by a dental wax-up, or imaging denture. Using computer produced digital stereolithographic printed guides, Dr. Bramanti is able to precisely place the dental implant into position with accurate angulation for outstanding success and the best long-term result.

CBCT-rendered dental implant positioning guides eliminate “surgical tourism” and improper dental implant angulation, thereby permitting the Board-Certified Dental Implant Expert to accurately deliver the dental implant into the ideal position for an optimal result.

Sinus Bone Graft

When the back teeth in the upper jaw are missing, the sinus cavity above toothless jawbone can collapse resulting in loss of natural bone. In a sinus bone graft procedure (or sinus augmentation), the sinus is accessed from the side and then lifted to its proper position filling the created space with bone graft material to promote bone growth inside the sinus cavity.

Sinus bone grafting and elevation involves adding bone into the space created in the sinus cavity thereby increasing the bone volume in this region to support dental implant placement (see below).

Extraordinary Bone Grafting Skills

Dr. Bramanti is widely recognized by Dentists and Dental Specialists as Fresno’s Bone Graft Authority with a superb record of extraordinary success. If you have lost teeth and jawbone, Dr. Bramanti is highly skilled at helping you regain your lost anatomy. With over 37 years of experience and outstanding success, his bone grafting capabilities are unrivaled, resulting in the complete restoration of lost bone and replacement of lost teeth with dental implants. When looking for bone loss solutions, consider the exceptional bone growing talents of Dr. Bramanti for replenishment of your bone to a natural state of volume, function, and well-being.

All-on-X

All-on-X is an alternative to placing a top and/or bottom set of replacement teeth, called a full arch or full mouth dental implant denture. Usually four to six dental implants are placed in available bone, sometimes without the need for bone grafting. Special abutments are used so that a temporary set of replacement teeth can be placed the same day or a few days later. You follow a modified diet while the gum tissues heal and the implants bond with your natural bone. After about six months, the permanent replacement teeth will be placed, and you can resume a regular diet.

Immediate Load Dental Implants

Also called “Same Day Implants” or “Teeth in a Day,” immediate load dental implants allow placement of a temporary tooth or temporary implant-supported denture during the same appointment as your dental implant placement. This may be an option if you have enough natural bone and a dental implant secure enough to support immediate placement and load and the bite does not cause too much pressure on the new temporary tooth or dental implant denture appliance.
